Intake Process:
Upon Admission:
o Responsible of the completion of the Inventory
o Ensure that staff completes the inventory at admission and that any missing items are made available to the child on the next business day or as soon as available
o Ensure that Intake/Discharge Inventories are placed in the intake’s file. All items must be locked at all times
o Update of the Intake Inventory while the child is in care
o Ensure the inventory is provided to the assigned CW upon release of the child.


Program expectations
o Discuss with the children Program expectations, Children’s Rights and Responsibilities Grievance letter and Values. This will be completed in conjunction with the Program Director
o Ensure that this information is given to the CW for filling
o Ensure that the children get a copy of this information
o Introduction to Behavioral Plan and PBIS (Gotchas and Check-in)


Portal Intake Assessment
o Completed with the child and in the portal within 24 hours of admission
o Sign and Print Intake Assessment (Place in child’s file or place in CW mailbox)
o Take a picture of the child. Uploaded in the Portal (Place in child’s file or in CW mailbox)
o Submit a signed Medical Consent Form to Michelle Flores in the Clinic for POH Binder
o Print Trigger Reports (Medical and Authorization of Placement) and bring them to the Program Director to be signed
o Send an email to POH informing of the completion of the Intake Assessment, utilizing the Intake table.
o For additional confidential information obtained during the intake, an email must be send to the assign Clinician, Case manager, Case manager Supervisors, Lead Clinician, Assistant Director of Clinical and Case Management, Assistant Director of Foster Care, Assistant Director of residential and Program Director.

Lewis and ORR Portal:
 Admit Children in the System - within 24 hours of arrival
 Discharge Children from portal as requested
 In Lewis Reflect children’s Movement as needed: Discharge, Transfers, Hospitalizations.
 Email CW and LCW with request for Child movement form once Travel/Discharge email has been received
 Send email to Assistant Director if Foster Care when the movements are completed


FTC Program
Weekly tracking of the following documents given to the Foster Parent:
o Weekly Phone cards
o Phone Call Log
o Recreation and Spiritual Logs


o Call Foster Parent on 1st or 2nd day of child’s placement to ensure receipt of Contact Folder and informational forms
o Based on the approved Phone Contact List sent by the Case Manager, complete the form sent to the Foster Parents. This must occurred within 48 hours of the child’s placement to the program
o Daily review of the completion of the Daily Sign-in sheet for drop offs and pickups of each UC
o Provide Foster Parent with an update Program Contact List.
o Upon the child’s approval for release from the program, a follow up call must be made to the Foster Home to remind them to send all pending forms or documentation.
o Coordination of Transportation in collaboration with the other SL on duty
and the Travel Coordinator


o Complete Orientation sessions with all new UC, within 48 hours of admission to the program.
o Upon completion of the Orientation session, update the Orientation spreadsheet in the POH, O Drive
o Send an email to POH informing that Orientation took place and include the date and name of all UC’s
o Bring Orientation packets to the “Orientation Box” in the Case Manager’s office


Recreational Program
o Create Monthly Recreational Calendar and post it Monthly
o Supervision to staff recreational facilitator who assist in creating Calendar of activities
o Ensure necessary supplies are available for activities listed
o Ensure that recreational program is being executed
o Ensure that calendar is posted and updated
o Weekly rounds to ensure compliance and identiify any barrier for success
o Meet on a weekly or bi-weekly basis with the Recrational team/club (both staff and chidlren)
o Maintain weekly review of Binders (Recreational, Spiritual) for appropriate completion of information
